- When you receive your laboratory kit from Private
Diagnostics, please read and follow all directions
carefully. Failure to follow all directions
supplied with the laboratory kit could significantly
affect the outcome of the results of your test. It
may mean that your test results are not accurate
or could result in a false negative report.
- Once you have received the laboratory kit, do
not collect the specimen until you are sure that
you can ship the box within 24 hours.
A delay in the shipping of your specimen to the
laboratory could significantly affect the outcome
of the results of your test.
- It is important that you always wash your
hands before collecting the specimen and
after you have finished. Please dispose
of all materials in the proper receptacle.
- Do not put any of the liquid in the transport
tubes directly on your skin or on any
other part of your body.
- Do not drink, swallow, inhale, snort or insert any
of the liquid from the transport tube into your body.
- Do not store your collection kit in the refrigerator
or the freezer. Please keep it at room temperature until
you are ready to collect your specimen and then immediately
ship it.
- The specimen collection devices, such as the transport
tubes, urine cups and other items are not
intended to be inserted into your body in any way.
Do not put these items into your ears, nose or throat.
The cotton swabs are not intended to be inserted
into the vaginal area at a depth to exceed 1 inch.
Swabs should not be inserted into the rectum.
- Private Diagnostics is not responsible
for any harm from the collection of any specimen
for laboratory testing. Ordering the kit
and then sending the collection kit to the laboratory
constitutes your acceptance of any responsibility
for any harm that results from the collection process.
- Always make sure to urinate into the urine
collection cup and then to transfer
the urine into the transport tube.
- The ordering of a laboratory kit from Private
Diagnostics signifies the acceptance of all responsibility
for any and all harm, damage or injury, which may
result from the collection of the specimen or the
knowledge of the results of the test. The consumer
acknowledges and agrees that Private Diagnostics
is not responsible for the negligent or intentional
acts or omissions of any healthcare supplier or provider
that the consumer is referred to or for any action
or inaction taken by the consumer; that the total
liability of Private Diagnostics, its officers, physicians,
directors, employees, agents and stockholders is
limited to the purchase price of any products through
Private Diagnostics. Private Diagnostics will not
be liable for any direct, indirect, special, incidental,
consequential or punitive damages.
- The accuracy of your results is dependent
upon your ability to collect an adequate sample
for testing. If an adequate sample is not supplied
to the laboratory, your results may be inaccurate.
- DISCLAIMER for HERPES TESTS :
a Negative Result or Not Detected Result does not
rule out the presence of PCR reaction inhibitors
in the patient specimen or Herpes Simplex Virus DNA
in concentrations below the level of detection by
assay. The performance characteristics of this test
was validated by ARUP Laboratories, Inc.
